Arena Decathlon
The Arena Decathlon event, more info HERE, was a series of 5 multi-day events, consisting of 10 different game play modes, spread out from December 18, 2021 through January 1, 2022. Each event cost 400 gems / 2,000 gold. Depending on the event, you would play to a total of either 5 or 7 wins, giving you the maximum reward payout plus a unique Decathlon token based on which of the 10 game modes you played. These tokens showed up in the top status bar of Arena next to where the Player Draft tokens would sit and were orange in color. Those that collected at least 8 different unique Decathlon tokens would be eligible to enter the Arena Decathlon Finals, which was held on January 8, 2022, for even greater rewards, including a grand prize of a complete set of Kamigawa: Neon Dynasty.
These sleeves all had very generic names, so we shall wait and see if they gain new names if they ever show up as a Daily Deal in the future.

These sleeves did not have a name attached to them. They are from the Secret Lair: Heads I Win, Tails You Lose. These are available as part of the first ever Secret Lair Commander Deck. The two sleeves are part of a new reversible card, where each side of the card is a different piece of art with different flavor text, yet it's the same card. - Counterspell
- Fabricate
- Mystical Tutor

Cathartic Reunion
Cathartic Reunion
This sleeve was a reward during the 2021 Thanksgiving holiday for a standard singleton event called The Gathering. The event was free, and you were able to use any standard card, even if you didn't have it. I see this as more data mining on WotC's part for meta info. Regardless, it's a free sleeve! The first win got you this sleeve. The second win got you an avatar of Halana and Alena.
Cathartic Reunion was first depicted on a card of the same name in Kaladesh, in which Chandra is giving her mother, Pia Nalaar, a hug. Illustrated by Howard Lyon.

Extra Life 2021
This sleeve was available during the Extra Life 2021 event, which is a charity event that benefits the Seattle Children's Hospital. The sleeve was available for a limited time from November 5-8.
The cost of the sleeve was 800 gems or 4,000 gold, with net proceeds being donated to the charity. The asset name in the game data for this sleeve was "extra life 2021 ajani".
I do find it funny/sad that they mention "net proceeds" ... on a virtual item. Really WotC/Hasbro?

Tentacle
Tentacle
This sleeve was found in the asset files after the Sept 16, 2021 update. There is never any indication to what they will be used for. Most are for event related rewards. This one happened to just be included in the daily deal for this day, which was all sea themed card styles.
It was available for 360 gems / 1800 gold (40% off)
Illustrator: Dan Scott
Dan Scott also illustrated the Nadir Kraken from Theros: Beyond Death. The Nadir Kraken created a Tentacle token, and is based on the same art style that he used for that card.

World Championship XXVII
World Championship XXVII
Worlds Trophy
Worlds Trophy sleeve was given out again this year in one of two ways.
If you participated in the Worlds Showcase XXVII event in Arena, your 3rd win would reward you with the Worlds Trophy sleeve.
Also, if you participated in the Find Your Champion event, and picked the winning player of the event, you would receive the Worlds Trophy sleeve, pet and 6 rare ICRs.
The World Championship XXVII sleeve was given out to those who picked 2nd-16th place winners in the Find Your Champion event.
Being that the Worlds Trophy sleeve was obtained previously by may people, and it was available by winning 3 somewhat easy matches in a free event, I see the World Champ sleeve being the more rare of the two since you won't get it if you happen to have picked the winner of the event.

Jack-o'-Lantern
Jack-o'-Lantern, or as the file name in the assets called it, Cerberus Pumpkin. Mousing over the bundle's "?" was the only place it gave the name of the sleeve Jack-o'-Lantern.
This sleeve was available from Oct 1, 2021 for a limited time.
It was bundled with a Pet, Pumpkin Cerberus, for 3000 gems / 15,000 gold, or you could purchase the sleeve on it's own for 600 gems / 3,000 gold.

Why is Arena Hiding the Daily Deal Sleeve?
As of the Sept 16 update, all sleeves or other non-card style items, are being put at the end of the daily deal list. They no longer show up on the main store page in the daily deal section as the showcase item.
The most obvious guess for this change is so it requires players to click the daily deal itself and scroll through all of the items to see if there is anything besides card styles available.
While this is nothing majorly game changing, it does add a slight inconvenience now. Players will need to be aware of this so they can make sure to click on the daily deal each day and scroll through all the items so they don't miss out on sleeves, avatars, etc.

Renewal 2021 Sleeve
This sleeve was part of the Renewal package that was rewarded for returning players prior to rotation. Once rotation happened, and the new Midnight Hunt Mastery was released, this sleeve was slotted in as the final returning player reward at level 31 on the Mastery.
It's file name was 'Bundle Insert", but on the Mastery itself, it's called Renewal 2021 Sleeve.
I am not yet sure what this art is from. It appears to be a custom piece for this purpose only and not from any card.

Sphere of the Suns - Streamer?
I have no idea what this Sleeve is from. It was sent to me by someone who pulled a bunch of image assets out of the game for sleeve art. The file name included the wording "basic streamer". I'm guessing this was given to streamers, but I have never seen it used on any streams I watch.
The card art is from Sphere of the Suns, originally printed in Mirrordin Besieged, and reprinted again in Modern Masters 2015.
If anyone has any info on this, I would greatly appreciate it. For all I know, it could just be a neglected asset in the game files that was never used.

Food Golem
Available for a limited time starting Dec 10, 2020 for 600 gems / 3000 gold.
This is the 2020 holiday sleeve, as their patch notes stated "Share some festive joy with the Food Golem Sleeve." I was hoping for something more "festive" than a giant monster covered in candy.

Black Lotus
The Black Lotus bundle was released on October 22, 2020 in the Arena store and included a Black Lotus pet and a Black Lotus exquisite sleeve.
The cost was 3,500 gems / 17,500 gold for the bundle. Individually, the Black Lotus pet cost 3,000 gems / 20,000 gold and the sleeve cost 1,200 gems. The bundle was an arbitrarily good deal if you wanted these two items at roughly 17% off the gem price. The gold price made the bundle an even better value since the sleeve was not obtainable with gold. This was an obvious gold sink.

Guild Sleeves
With the March 2019 update came these Guild symbol sleeves. These were available during the Guilds of Ravnica and Ravnica Allegiance sets in the Arena store.
There were two sleeves for each of the 10 Ravnica guilds, both a plain boring sleeve and an exquisite sleeve.
4/18/21 Update: Regarding the "plain" sleeves, after finding them in the game asset files, each of these sleeves are actually called "metallic" sleeves.

Valentine's Day
This was a colorful card back that was found in the game's asset files.
There was a mention of it in Twitter from the designers HERE.
Not much is known about it. I assume it was a card back that showed up on the day of, and for that day only, much like other one-time-only card backs on certain holidays.

Early 2019
Embossed Mana Symbols
These were one of the early sleeve bundles available on Arena, which players could purchase in the Arena store until May 23, 2019. They were available for 10,000 gold for all 5, making them one of the most expensive and rare sleeves available due to it being harder to farm gold at the time. Now we really don't think too much of 2,000 gold per sleeve.
- Embossed Green Mana - 03/17/2021 Available as a Daily Deal for 240 gems / 1,200 gold.

Miscellaneous Sleeves
Country Flags
WotC Gives out flag sleeves to live streamers as well as codes for them to give the same sleeves away during their live streams. You don't see these very often except typically during large streamed competitions.
5/16/20 Update: These flags were later released on the Arena Daily Deals for 1,500 gold / 300 gems each. This brought the average price of sleeve down from $5.63 to $4.30.

Arena Metallic (Unused Asset)
This doesn't appear to be used in the game anywhere. It's name is "Metallic Arena".
My guess is this is done in the same way that the "Basic / Metallic" guild symbol sleeves are. There is a lighting/glow effect that shows through the dark areas on the card, such as the logo, and gives the card a somewhat shimmer and the logo glows. It's not an exquisite though.
I don't know when this was introduced into the game, but my guess is it was one of the first early sleeves. If I find out otherwise, I'll update this. Please let me know if you have any info on this.

Cookie: 2018 Holiday Pauper Event
The Cookie "sleeve" is actually just an alternate card back used during the Holiday Pauper Event from December 2018.
This is the event update posted from WotC from December 2018:
Holiday Pauper
"Happy Holidays from the MTG Arena team! Bring your deck of only commons and enjoy the winter scenery as you play!"
The image was found in the game's assets, and is simply titled "cookie".

Alpha/Beta Card Backs
Closed Beta Test Card Backs
This default looking card back was similar to one used during the Arena Closed Beta period. The MTG logo was in yellow, and they had removed the Deckmaster wording from the bottom of the card.
